You control a shinobi, Joe Musashi, who must battle his way through each level in search of hostages which must be rescued. You have shurikens, kicks and punches to defend yourself with, as well as powerful ninja magic which can clear the screen of enemies in one go. Members of the gang and at the same time the criminal organization Zeed kidnap the children of Musashi's clan. Without batting an eye, Joe enters the gang-controlled area and uses martial arts to fight his way to the very heart and headquarters of the clan. Here he confronts its leader himself and uncovers the shocking identity of Nakahara Musashi. You as a Joe Musashi traverse a set area in five missions, and at the end of each you face a boss that is always introduced to you before the start of each level. First, you walk through a slum with a gigantic samurai named Ken Oh; in the harbor, the Black Turtle stands in your way. In a secret base that is hidden in a cave, you must destroy a fire-spitting statue of Mandara. The ninja training camp is headed by the huge samurai Lobster. The last part takes place already in the gang's headquarters, and here Joe faces the masked ninja Nakahara. Each mission consists of several additional levels in which Joe frees the hostages and kills all the ninjas who attack him. The game allows both close combat, where the player uses kicks and punches, and it is possible to use cold weapons, especially katana or shurikens.
